Make sure all the information is filled out on the form(s).

Be sure to have a supervising person sign your forms and provide contact information.

Turn in your forms as soon as you have them filled out. Don’t procrastinate!

Remember, you can get credit for both the graduation requirement and the
Saints Community Connections progam at the same time, so fill out both forms!

Remember to ask your counselor how many hours of community
service
you have turned in. You need a minimum of 40 before you graduate!
