How To Turn In Your
Community Service Hours
It only takes four easy steps!

1. Gather the required form(s) from the counseling center. (If you need help, ask Mrs.
   Pass in the counseling center. She will be more than happy to help you.)
   
   
2. Fill out your personal information on the form(s) (including your name, graduation year, address, etc.)
   
   
3. Take your form(s) with you to the place where you will be completing community service
   to have the information filled out, OR have the supervising person fill out the form(s) after
   you have completed your hours.
   An alternate to this step is to have the volunteer manager, or other supervising person, write a letter,
   signing it at the bottom and providing contact information. (The basics of the letter could include
   information about where you completed your volunteer hours, what you did when you volunteered, etc.)
   
   
4. Take your filled out form(s) back to school, and turn them in to your counselor as soon as possible.
   After two years, you will not be able to receive credit for them. Turn them in as soon as you have
   them filled out, so you won’t lose the forms.
