Application Instructions:
Thank you for taking time to complete the Bellevue School District Volunteer Application. We welcome you and appreciate your willingness to volunteer. To ensure the safety of our students and to be in compliance with Washington State Law (RCW 43.43.830), we ask that you follow the directions below. There is a growing awareness that this screening process is not only prudent but an effective safety net for our volunteers as well as our students. Please note:
Washington State Patrol form, please complete: Section C (do not include your social security number) and Section D (no finger prints are needed). Return with application, do not send to Olympia.
Include a copy of photo identification: valid Driver’s license, State Identification card or passport. (ID must include birth date).
Please return filled out form to the Medina Elementary school office.
